New Students

Welcome you to our district. Together, as a team, our district strives to provide a healthy educational environment for your student(s) to succeed. In accordance with The State of Illinois Regulations and our Board Policies.

Please note

Physical Exam & Immunization:

  • Students entering an Illinois school from out of state or out of country for the first time at any grade level must also provide the physical exam form.

  • The exam must be completed no earlier than a year (365 days) prior to the start of attendance at an Illinois school.

Oral Health Exam:

  • Students entering an Illinois School from out of state or out of country the oral health exam is only necessary to be completed if entering at the grade levels listed (K, 2, 6, 9).

Eye Examination:

  • For students entering an Illinois School from out of state or out of country the exam is required to be submitted
In State Transfer/returning Student(s)

New Student(s) register after October 15th of the current school year shall have 30 days, following registration to submit the following up-to-date documents.

*unless an exception or extension applies, the failure to comply will result in the student(s) exclusion from school until the required forms are submitted:

Student emergency action plans guide our health care team to provide quality health care in our schools, account for student safety, and protect a student’s rights while reducing any health barriers to the learning environment. These plans facilitate communication between the student’s family, outside health care providers, teachers, administrators, and the school’s health care team to ensure that the student's access to the classroom is maximized in a safe environment. The plan defines the criteria of an emergency for a given diagnosis and allows treatment protocols to be formally established. It is helpful for our Registered Nurses and Licensed School Nurses to receive any plans from physicians prior to the start of the school year or as soon as possible after a physician has made a change to an emergency response plan, so our staff has time to review and clarify the information.

Out of State Student(s)

Out of State Student(s) transferring into an Illinois School District who does not have the required proof of Immunization by October 15th may attend classes ONLY with proof that an appointment for the required vaccinations is scheduled with a 30 day grace period from starting classes. After 30 days, student(s) may no longer attend classes till proof of vaccination is properly submitted and filed.
