The athletic (sports) physical assessment is required to be on file prior to try-outs for extracurricular or interscholastic sports in grades 7-12.

• Illinois School Physical Certificate of Child Health Examination may be used for this purpose if it has been completed during the past twelve months.

• An IHSA Pre-participation Examination Form is the preferred form and is acceptable for 13 months from the date of the examination.

• Either acceptable examination should remain current through the end date of the sport. Should either exam expire during the sport’s season, a new examination will be required to continue participation in the sport.
