The Williamsville-Sherman School District is partnering with the Illinois Department of Public Health to make COVID-19 vaccines available to our students & community.
We will host a voluntary COVID-19 vaccination clinic for students ages 5-11.
When: Wednesday, Dec. 15th from 2:30pm-5:00pm (1st Dose) & Wednesday, January 5th from 2:30pm-5:00pm (2nd Dose)
Where: Sherman Elementary School Gym (park in the east parking lot at Sherman Elementary and enter through east doors to gym)
How: Please use the link below to reserve appointment time (participant names are not viewable on sign-up)...reserved time will be the same for the 1st & 2nd dose.
***If more than one member of your family needs a vaccine please select the total number of appointments needed when signing up for that time slot.
Anyone under the age of 18 needs to be accompanied by a parent/guardian and consent forms will be available on site. Please consider consulting with your local doctor regarding vaccinations.
